- Access the Coordination Request Form: Click on the “Coordination Request Form” link.
- Select the Appropriate Category: Choose the category that best fits your request (e.g., New Frequency, Update Existing Frequency, or Decommission Frequency).
- Complete the Form: Carefully fill out the entire form with the required information. It is crucial that all fields are completed, as missing information may delay or result in the rejection of your submission.
- Submit the Form: Once the form is filled out, click on the “Submit” button.

- Processing Time: Please allow up to 30 days for a frequency to be issued. We strive to process requests as quickly as possible, but thorough reviews are necessary to ensure accurate coordination.
